I have done 6 treks with Ramesh Adhikari(Adventures in Nepal) over the last 10 years.
Most of these treks have been customized itineraries that we worked together on in the months leading up to my trips to Nepal.Ramesh has been able to create an itinerary that includes appropriate acclimation and a daily hiking time that suites my fitness levels and the season/regions weather patterns (avoiding afternoon rains for example). But at the same time including the regions I prefer to cover on a trek.
We have done treks between 20 days and 30 days long.We have been to a number of less travelled places such as Tsum Valley, Mesokanto La, Pungyen Gumpa, Nar-Phu Valley(including Kang La), Gosaikunda Lake/Helambu trail(following Langtang trail) and many others.

He was an amazing guide and it was an amazing experience. You don't have to be a trained athlete; It's not climbing Everest and I wasn't in the best shape and still managed to have the time of my life.
